Forms

The following ACS forms are necessary for publication: Copyright Status Form, Permissions Verification Form, Permissions Request Form (provided permission is necessary), Complimentary Copy recipient Verification Form, and the Manuscript Submission Form. IMPORTANT: All forms should be submitted to the editor with the final, camera-ready chapter; the forms should not be submitted directly to ACS.

Copyright Status Form in PDF format*
Please complete the Copyright Status Form by filling in the top portion and by signing on the appropriate line or by having an authorized agent of your employer sign. Please submit this form with the final, camera-ready version of your paper. We cannot publish your chapter without receipt of this form.
Permissions Verification Form in PDF format*
We must have a signed Permissions Verification Form for each paper accepted for publication. If all tables, charts, figures, and photographs are original, please indicate this by signing the Permissions Verification Form in section A. If all art is not original, please sign the form in section B.
If you have signed the Permissions Verification Form in section B, complete the Permissions Request Form to obtain appropriate permission from the copyright owner (usually the publisher of the book or journal in question).
Complimentary Copy Recipient Verification Form
A maximum of three (3) complimentary copies per chapter will be given to chapter authors. No author shall receive more than one complimentary copy. Complete the Complimentary Copy Recipient Verification form to ensure that complimentary copies will be sent to the appropriate authors.

Use this checklist when you submit the revised, camera-ready version of your manuscript.
